Fluence, Spot Size, Pulse Width: How to Read Your Own Treatment Settings
You are paying for four numbers you probably never see.
Every laser hair removal session comes down to a handful of parameters that a technician enters before the first pulse. Those numbers decide whether the follicle takes enough heat to stop producing hair, and whether your skin takes so much that it blisters or changes color. Everything else, the robe, the eye protection, the playlist, is packaging around that decision.
Almost nobody explains the numbers to clients. Partly because it takes time, partly because a client who understands settings is a client who can tell when a clinic is treating too conservatively to be effective. I think that is a bad reason to keep people in the dark, so here is the whole thing.
The one principle underneath all of it
In 1983, R. Rox Anderson and John Parrish published a paper in Science describing selective photothermolysis, the idea that a suitably brief pulse of light absorbed preferentially by a pigmented target can destroy that target while sparing the tissue around it. Every hair removal laser in existence is an application of that paper.
The pigmented target is melanin in the hair shaft and follicle. The tissue we are sparing is your epidermis, which also contains melanin. That is the entire problem in one sentence: we are aiming at pigment, through pigment.
The settings exist to widen the gap between the two. Enough energy into the follicle, not enough into the skin above it.
Wavelength: which laser is firing
The GentleMax Pro Plus houses two lasers in one handpiece. The 755nm Alexandrite is absorbed strongly by melanin, which makes it efficient on hair but also means more of it is absorbed by pigment in the epidermis. The 1064nm Nd:YAG is absorbed less by melanin overall, penetrates deeper, and passes through epidermal pigment more safely, which is why it is the wavelength for Fitzpatrick IV through VI.
That choice is the single largest safety decision of your treatment, and we have written about it at length in our comparison of the GentleMax Pro Plus and diode platforms, so I will not repeat it here. The short version: if a clinic owns only one wavelength, that wavelength gets used on you whether or not it suits your skin.
What matters for this article is that wavelength changes what every other number should be. A fluence that is appropriate at 1064nm is not appropriate at 755nm.
Fluence: how hard the pulse hits
Fluence is energy per unit area, written in joules per square centimeter. It is the closest thing to a volume knob.
Too low and the follicle warms up without sustaining permanent damage. The hair sheds a little, regrows on schedule, and you conclude that laser does not work on you. Too high and you deliver more heat than the epidermis can shed, which is how you get blisters, or the pigment changes that take months to resolve.
Undertreatment is by far the more common failure, and it is invisible in the moment. Nothing goes wrong. You feel a mild session, you leave happy, and eight sessions later you have paid for a course that never reached therapeutic energy. It is the reason people show up at our consultations having spent close to two thousand dollars elsewhere with most of their hair intact.
Why we are not going to publish a target fluence
I considered putting a table of numbers here and decided against it, for a reason worth explaining.
Fluence is not meaningful on its own. It is only interpretable alongside wavelength, spot size, pulse width, your Fitzpatrick type, your hair caliber, and how your skin responded to the last pulse. A number that produces a perfect result on one person can burn the next one, and the same number means different things on different machines.
If I hand you a figure to wave at a technician, I have given you a way to pressure someone into settings that may be wrong for you, and I have done it from a distance without seeing your skin. That is worse than telling you nothing.
What I can give you is something more durable: the direction each variable moves, and the questions that reveal whether your clinic is reasoning or guessing.
Spot size: the variable nobody explains
Spot size is the diameter of the beam where it meets your skin, in millimeters. The GentleMax Pro Plus runs spots from 1.5mm up to 24mm, with a 26mm option.
Most people assume a bigger spot is just faster, covering more ground per pulse. That is true and it is the smaller half of the story.
Light scatters as it travels through skin. With a narrow beam, photons scatter sideways out of the treatment column and are lost before they reach follicle depth. With a wide beam, photons scattering out of the center are replaced by photons scattering in from the surrounding beam. The wider the spot, the less energy leaks away, and the deeper the effective penetration.
The practical consequence is that identical fluence at two different spot sizes does not deliver identical energy to the follicle. A larger spot at a given fluence reaches deeper than a smaller spot at the same fluence. This is why comparing your settings to a friend’s is close to meaningless unless you also know the spot size, and why a clinic that treats a full back with a 10mm spot is doing something slow and probably shallow.
Pulse width: how fast the energy arrives
Pulse width, also called pulse duration, is how long each pulse lasts, measured in milliseconds. The Pro Plus ranges from 0.25ms to 100ms.
The governing concept is thermal relaxation time, which is roughly how long a structure takes to shed half its heat. Deliver energy faster than the target can cool and heat accumulates inside it. Deliver it slower and the heat diffuses out into the surrounding tissue while you are still delivering.
A hair follicle is large and cools slowly. Your epidermis is a thin layer and cools quickly. That difference is the opening we work in. Stretching the pulse gives the epidermis time to shed heat between the beginning and end of delivery, while the follicle, cooling more slowly, holds onto it. Longer pulses are therefore gentler on skin, which is why they are used on deeper skin tones.
The tradeoff is that fine hair also cools quickly, so a long pulse lets a fine follicle dump its heat before it accumulates damage. Short pulses treat fine hair better. Candela markets the 2ms setting on this platform specifically for fine and residual hair, which is the sort of thing that matters in your last two sessions rather than your first two.
Cooling: the setting that lets the others be higher
The Dynamic Cooling Device sprays a burst of cryogen onto your skin a fraction of a second before each pulse. It has its own two numbers: spray duration and delay, both in milliseconds.
Cooling is not a comfort feature, or not only that. By dropping the temperature of the epidermis before the pulse arrives, it raises the ceiling on how much fluence can be delivered safely. Adequate cooling is part of what makes an effective fluence possible. Insufficient cooling forces a technician to treat conservatively, which brings you back to undertreatment.
If your session feels sharply hot rather than cold-then-snap, say something during the session rather than after.
What should change over your course, and what should be recorded
Your settings should not be identical at session one and session six unless someone can explain why.
As a course progresses, the remaining hair gets finer and lighter. Finer hair is harder to affect, which usually means the plan has to adapt: shorter pulse width, sometimes higher fluence, sometimes a different wavelength for the last sessions. A technician who runs the same numbers every visit is not responding to what your skin is telling them.
Every setting used on you should be written in your chart at the time. Wavelength, fluence, spot size, pulse width, cooling, number of passes, and the skin response observed. That record is what makes session seven a decision rather than a guess, and it is what any other clinic would need if you moved.
You are entitled to see it. Ask.
The endpoints we are actually treating to
Settings are inputs. These are the outputs a technician watches for, and knowing them lets you evaluate your own session.
During treatment, we look for perifollicular erythema and edema: slight redness and a small raised bump around individual follicles, appearing within a few minutes. That is the visible sign that energy reached the follicle. Uniform redness across the whole area without follicular definition is a different and less useful signal.
In the one to three weeks after, treated hairs push out of the follicle and shed. Shedding is the clearest evidence that the session did something.
What is not a reliable endpoint is pain. Discomfort correlates loosely with energy, and plenty of people tolerate effective sessions comfortably. But a session with no sensation at all, no perifollicular response, and no shedding afterward is three absent signals rather than one, and that combination is worth asking about. Four questions worth asking any clinic
Not gotcha questions. Questions where a competent answer is easy and an incompetent one is obvious.
Which wavelength are you using on me today, and why that one for my skin type? A clinic with a dual-wavelength platform should answer instantly. A clinic with one wavelength will tell you something vaguer.
What were my settings today, and are they in my chart? The answer should be yes without hesitation.
What did you change from last time, and what were you responding to? This is the one that separates a protocol from a person paying attention.
What did you see on my skin during the session? If the answer is perifollicular edema, good. If the answer is nothing in particular, ask what they expected to see.
Frequently asked questions
What fluence should be used for laser hair removal? There is no single correct number. Fluence is only interpretable alongside wavelength, spot size, pulse width, your Fitzpatrick type, and your hair caliber, and the same figure can be ideal for one person and unsafe for another. Ask your clinic what they used and what they were responding to, rather than asking for a target to hit.
How do I know if my laser settings are too low? The signals are a session with no sensation, no perifollicular redness or small raised bumps around follicles afterward, and no shedding in the one to three weeks that follow. Any one of those alone can be normal. All three together is worth raising.
Does a bigger spot size mean a better treatment? It means deeper effective penetration at the same fluence, because less light scatters out of the beam, and it means faster coverage. It is not universally better. Small spots exist for precision work around the lip, chin, and other detailed areas.
Should my settings change between sessions? Usually yes. Remaining hair gets finer as a course progresses, and finer hair generally needs a shorter pulse width and often different energy. Identical settings across an entire course, with no stated reason, is worth questioning.
Can I ask for my treatment records? Yes. Every setting used on you should be documented at the time of treatment, and you should be able to see it. If you move clinics, that record is what lets the next provider start from evidence instead of guessing.
Why does it hurt more on some areas than others? Skin thickness, follicle depth, and nerve density vary by area, and settings are adjusted per area accordingly. Pain is not a proxy for effectiveness in either direction.
